Retirement village adopts health
record portal
WATERLOO, Ont. –
After a successful pilot testing phase, Luther Village on the Park
retirement community in Waterloo, Ont. is adopting MedManager’s health
and chronic disease wellness portal as its primary resident healthcare
management program.
Since April 2007, Luther Village has utilized MedManager’s portal to
engage and care for residents living with diabetes and heart failure.
Now, Luther Village and MedManager will implement the next generation of
the portal to create a health record and care plan for all residents,
which will be based on intake and assessment tools incorporated into the
online system.
Initially, staff will utilize the system, enabling multi-disciplinary
collaboration and timely access to important resident health
information. In 2011, independent residents will be offered a secure
personal account where they can access their health record and a suite
of modules for chronic disease management.
Other tools will facilitate secure communication between the resident
and the Luther Village healthcare team, their informal caregivers, and
their peers. Residents can log into their account at the Luther Village
Wellness Centre, private residence, or anywhere they have access the
Internet.
“We understand the importance of providing an environment where
residents can enjoy good health and participate in activities and
services that will maximize their wellness. We believe that the
MedManager team and portal will help us achieve that goal,” said
Jennifer Hesson, Director of Seniors Services at Luther Village. “In
2008, the Ontario Association of Non-Profit Homes & Services for Seniors
(OANHSS) honoured us with their innovation Award in recognition of the
diabetes pilot project we did with MedManager,” added Hesson.
“The Luther Village experience has demonstrated that age is not a
widespread barrier to the adoption of online health management programs.
We are very excited about Luther Village’s healthcare team and residents
benefiting from our portal,” said Michael Cowan, President and CEO of
MedManager.
According to the Health Council of Canada, nearly 75% of people aged 65
and over live with at least one chronic disease such as diabetes, heart
disease, and Chronic Obstructive Pulmonary Disease (COPD).
