MONTREAL – A new policy of not interrupting nurses when they’re at medication carts or doing their charting has led to a reduction in medication errors of 60 percent at the McGill University Health Centre. The multi-hospital organization has attempted to create a ‘wall of silence’ around nurses who are busy with these tasks.

HALIFAX – The Nova Scotia government has followed through on a promise, posting its medical mistakes registry on the health department website, CBC News has reported. According to the government website, making the information public “raises the level of accountability – and demonstrates a commitment to transparency and openness. The goal is to share lessons learned and prevent the event from happening again.”

HALIFAX – Dr. Mark Fleming (pictured), Team Lead and Dr. Andrea Bishop, Co-Team Lead are undertaking an inter-disciplinary research study to gain a better understanding of patient and provider experiences and to identify best practices to improve safety in the transition between primary care and home care. The $45,000 study is being funded by the Canadian Patient Safety Institute, Accreditation Canada, and the Canadian Home Care Association.
