As patients demand greater access to their health data, and healthcare professionals require seamless integration of information to provide high quality care, the need for healthcare organizations to share information only intensifies.
Fragmentation of care is not unique to Canada – it is the case to a greater or lesser extent in all regions of the world – and it reveals itself in many ways. A lack of communication between specialists and primary care physicians is one of the most frequent and obvious negative impact on patients. Care coordination should be targeted to those who are most at risk of falling through the cracks.
Digital health technology provides an opportunity to improve the existing disconnected system and significantly enhance patient care. When health care providers and patients have access to complete information, including an integrated care plan, we can realize the full potential of a streamlined, coordinated healthcare system.
