Transform SSO recognized for work in homelessness

Lyn BaluyotWINDSOR, Ont. – IDC Canada and CIO magazine have named TransForm Shared Service Organization as a CIO Awards Canada 2023 award winner for their work on a proof of concept for developing a two-way health record from acute care facilities to primary care providers for unhoused people in our communities.

The objective of the concept is to improve access to primary and urgent care and the creation of a two-way health record from acute care to primary care by expanding the use of Oracle (Cerner) health information system through adoption of the ambulatory module within clinics. This collaborative effort is making strides towards providing vital primary healthcare services to those at risk of or experiencing homelessness and supporting the safe transitions of care as the individual navigates between acute care and primary care.

Every year, CIO recognizes the tech world’s most innovative organizations and accomplished leaders. This year would be the second for this popular Awards Program in Canada where CIO Awards Canada have recognized innovative Canadian tech projects and teams.

“We’re excited to celebrate CIO Awards Canada 2023, which honours Canadian IT organizations for projects driving digital business growth through technology innovation. Many projects are related to customer relationship management, data transformation, workforce management and data governance,” said Lars Goransson, managing director, Canada, IDC.

Executives from the winning companies will be featured at the CIO’s Future of Digital Innovation Summit & Awards Canada on November 15 – 16, 2023.

“We are pleased and proud of the work our team, led by Jennifer Smith, manager, Clinical Informatics, put in to this project. The two-way health record concept leverages the Erie St. Clair region hospitals’ instance of Oracle Health (Cerner Millennium) Health Information System (HIS) and the IT/IM services provided by TransForm to our member hospitals and community health organizations,” Lyn Baluyot (pictured), TransForm president and CEO said.

“Expanding the use of the hospital-based platform to allow access to the Electronic Medical Record (EMR) by health service providers in the community is a service TransForm provided to its customers and community partners and is an important part of its mission and values in providing better solutions and better care through digital health transformation.”
